For my manual Penis Extension exercises, before doing this exercise, rest for at least 1 minute before proceeding; start by having your dick in a less-than-hard state, then grab you glans base as in MPE2, but this time you have to reach for it from behind your legs/back and start pulling your cock in a BACKWARD direction.
